Super proud to announce that These Hands Academy is being recognised as a leader for implementing tech-enabled care solutions, enabling people to remain in their own homes because of early decision-making. Their programme has demonstrated sustained impact across Leicester, Leicestershire and Rutland, delivering measurable system savings. The initiative is now entering its third year.
Crucially, Connected Care aligns closely with the ambitions of the Neighbourhood Health Service by shifting care closer to people’s homes, strengthening the capability of neighbourhood-based teams.
The programme focuses on managing deterioration as core practice, not technology adoption, enabling care-home staff to recognise, escalate and respond to risk using a shared clinical language across health and social care, both verbally and digitally, using a common language that is recognised across the whole system.
Using the technology as an enabler, this approach has improved confidence, consistency and decision-making at neighbourhood level, supporting multidisciplinary working and reducing reliance on reactive services.
More than a local achievement, the Connected Care Programme offers a pioneering model for transformation. As health and care systems nationwide shift toward community-based, integrated delivery, the LLR experience provides a scalable, data-driven, and person-centred blueprint for building safer, smarter, and more equitable care ecosystems.
