Impact that lasts

Empower, Encourage, Enable

Compass showing change

Our Mission

Our mission is to ensure that nobody deteriorates unnoticed, by empowering people with a voice that is heard, valued and acted upon.

Always aiming to make learning simple, communication direct and outcomes impactful.

We exist to nurture the people behind the care, equipping carers, communities and changemakers with the skills, confidence and tools they need to thrive. Rooted in lived experience and co-design, our values are:

  • Compassion: People before process

  • Co-production: Listening, learning, action together

  • Courage: Leading change with heart and humility

  • Equity: Fairness and access in everything we do

Our vision is to change social policy, enabling people to remain well in the place they choose by maximising every intervention, leveraging technology, and utilising data to inform future planning.

Our approach

Every personal intervention opens a window of opportunity to look into one’s health and well-being. â€‹ Our approach is rooted in the values of the Darzi report, supporting proactive, person-centered care from hospital to home and from sickness to prevention.

For us, that means managing deterioration early, connecting services seamlessly whilst helping to build trusted relationships across services that support the shift from hospital to home, analog to digital, empowering early intervention and promoting equitable, compassionate communities.

Our Connected Care Programme demonstrates the art of the possible through working with teams to simplify processes, direct communication, and make impactful change that improves outcomes to ensure no one deteriorates unnoticed.

"The programme has fostered integrated and inclusive care by equipping care staff with targeted training and enabling the ICB to enhance services through data-driven decision-making. These Hands Academy plays a vital role in bridging the gap between health and care practice, technology, and community." LLR ICB

Creating system-wide efficiencies, evidenced by qualitative and quantitative analysis, our programme demonstrates

  • Reductions in avoidable harm

  • Improved communication across services

  • Stronger networks of care

Partnering with Universities, we are validating our impact and enabling a new generation to think about what future services could look like.

I founded These Hands Academy to equip health and care staff with the tools and training to deliver safe, proactive care by using a common language both verbally and digitally.

With over 30 years’ experience in the NHS, I have led major Local and National transformation initiatives, combining virtual and face-to-face education, digital integration and stakeholder engagement to drive system-wide lasting change.

Meet Sarah & Caroline

Sarah is key to our organisation and brings her experience alongside her compassion to support all of our members, helping them through the onboarding process, keeping everyone informed, and helps me to keep everything shipshape in the organisation too.

Caroline, our data analyst, brings the data to life, managing the technical side of understanding the nuances in various data sets to enable us to demonstrate our impact.
