The Connected Care Programme evaluation 2025 - From Pilot to Scale

I’m delighted to share that our Connected Care Programme – Pilot to Scale Evaluation 2025 is now complete and available to read.

Over the past year, together we’ve:

• Expanded our reach to over 100 care homes, connecting more than 3,000 residents to the system.

• Recorded thousands of NEWS2 scores and baseline observations, strengthening proactive care and improving patient safety.

• Achieved significant system savings through early intervention, helping people remain in the place of their choice for as long as possible.

• Gained National recognition at the HSJ, LGC and International Care conferences.

The evaluation reflects not just the data, but the voices and experiences of the people and organisations who have made this journey possible. Your commitment, feedback and innovation have helped us refine our approach to keep it simple, direct and ensure it remains truly impactful.
